Operation bounty reward bugs?

Some friends and myself ran the Operation's basic bounty 3 times earlier tonight. Every time we ran it, we got the same 4 rewards:
- 100 Kuva
- Vigilante Pursuit mod
- 300 Endo
- 100 Kuva
I can understand maybe getting these twice in a row, but getting these same 4 rewards 3 times in a row seems like the reward tables are completely bugged.
Has anyone else seen this bug?
