I'm doing Task 1 for this class and have had it sent back twice now. With different things wrong each time?

This is supposed to be an audit telling a hospital if they are ready for their joint commission accreditation audit and how to be ready if they are not.

I've done these 1000 times at work, so obviously I know how to do it.

My problem is that it's different things wrong?

The first time it was

"Appropriate recommendations, including policies and procedure changes, are identified in the action plan. However, the submission, lacks sufficient detail and support typically found in corrective action plans to ensure accountability, timely implementation of recommended, actions, and monitoring and control efforts."

So I added a new paragraph and explained more into how the hospital should correct each thing and gave a specific timeline.

Now it's returned again because I didn't say who was responsible for each task.

Just makes no sense to me since if I don't work at the hospital I wouldn't know who they are assigning what tasks to? I would assume senior leadership at the hospital (which is who I am supposed to be sending this audit to) would take the recommendations and disperse amongst themselves to correct the policies?

Am I looking at this the wrong way? I'm getting annoyed now so it definitely isn't helping my thought clarity.
