Are there templates or guidelines for personal statements from family?

I asked my sister to write a personal statement as supporting documentation for a VA claim (for OSA), and she asked me if there's a template or guideline she could follow. She also asked if there is a form she could fill out instead?

I told her that as far as I know it's just dated/signed a personal statement from her explaining symptoms she's noticed, how they affect me, and whether or not I had those symptoms before I joined the military-- that it wasn't necessarily a formal document.

For context, I didn't seriously consider the possibility of having sleep apnea until my sister told me she could hear me snoring through a few sets of walls, and I had my sleep study last night at a VA facility. The tech said that he couldn't discuss results exactly but I meet the criteria for "severe" sleep apnea, and they had to put a CPAP on me a couple of hours into it.
