Can’t keep working—what do I do?

I’ve been at my job a few years now but my mental health has become a real obstacle to my profession. I feel too overwhelmed and trying to hide or disguise my issues only makes it worse because I feel like the company is always watching me for a fuck up.
I’ve told them about my problems but it’s a small company and they really don’t have the manpower of financial freedom to be supportive.
I don’t know what answers I’m looking for here but any advice would help
