Surgeon put "possible methamphetamine use" in record, how to handle?

I am going to be getting some surgery on my hand and the surgeon told me told me that I needed to get some blood work before surgery.... well reading her notes she had written "His appearance also is concerning for possible methamphetamine" and the "blood test" is a drug test.

In 15 years of going to this VA hospital, she is the first one to do this.

Reviewing the notes, she has made this decision because I am thin, have bad teeth, "mildly hyperactive and talkative", and my "appearance".

Meth use - I have never done meth. I am on 11 prescription medications and the only non-prescription medications I take are OTC things like excedrine/cold medications and VERY occasionally CBD to break migraine pain cycles with the full knowledge of my VA neurologists/Seizure team (generally less than 0.35grams year, legal where I am).

Thin - Because of many medical issues I live a very sedentary lifestyle and for years I was obese. Since I can't cant really exercise (I had a seizure just walking around a store yesterday) it took a LONG time (years) to get down to my current weight (5'10" 175 pounds, at one point was at 240). Primary doctor is happy with my weight.

Teeth - Yes I have bad teeth. Combination of genetics (so did my father and others in my family except my brother who IS a meth head), lack of dental care, chronic low D3 (needed for teeth) and severe GERD (eats at enamel).

Appearance - After a bad seizure in 2019 and developing brain damage in the pre-frontal lobe, which effects the excecutive functioning area of the brain, I have let my overall appearance go. This is a common symptom of this type of brain damage and My g/f has to occasionally remind me to do self care. Since I worked remotely for over a decade I do have long hair and since I haven't worked since 2019 I did let my beard grow out. But when I went to the appointment I was wearing clean clothes, hair was combed (beard a little wild but I like that) I feel that my overall appearance was ok.

Mildly hyperactive and talkative - This was really a combination of me having one of my rare "low pain days", my normal personality coming through, and a side effect of my brain damage.

I do know that my overall appearance may rub some people the wrong way and I am used to doctors giving me the "do you use any illicit drugs" question who I give the same response as above and they are happy with my responses, but she never even asked.

My concern is that many doctors only read recent notes (like she did not see I was previously obese) having what she wrote on my record could affect my future care.

Currently she is the only one at the local VA that can do the surgery I need on my hand so I am a bit worried about doing anything, so what would be the best way to handle this?

I was thinking of sending her a note via secure messaging explaining things as I have done above but not sure if that is the best way?

EDIT:

Did forget to add, since 2022 the g/f and I are "technically homeless" living in a barely functioning travel trailer (no running water in it/barely have electrical) in her sons backyard after losing our home. My clothes may have looked a little threadbare/worn because we don't have much money to buy new clothes the last few years.
