ULPT - how do I keep Medicaid from taking my mom‘s house?

My mom needs to move home because she can’t be alone anymore. We have a 37 foot fifth wheel in our backyard that I would like to buy from a friend for her to live in. Her house is only worth about 87K & she says if we try to sell it Medicaid will take most of that for her hospital bills and food stamps. Is there a way she can gift it to me so that I can sell it buy her the camper?

Edit/update: You people are an awesome combination of informative & Hilarious! Thank you so much! Update - We called the County Clerk & it doesn’t appear that there’s any type of lien on the house yet. We have an appt with a good Elder Attorney.
