Are there really Italians that don’t understand English?

Keep in mind I’m from the Philippines, which is a non-native English speaking country in Southeast Asia, but here even the most rural Filipino can somewhat understand English. Which is why I found it so hard to believe that certain Italian characters like the Sicilian Di Grasso women or Quentin’s boat captain couldn’t seem to understand English at all. It’s literally an international language and the language of the world.
