It seems awfully inefficient and time-consuming for the Fire Nation War Council to direct movements of individual army divisions Shouldn't that be the responsibility of lower-ranking field commanders on the war front?

I think it's more likely that they confused military naval captaincy (a specific rank with different responsibilities in different contexts) with civilian naval captaincy (dude in charge of a boat) and just knew it was military terminology.

This also isn't consistent across countries, cultures, and time periods. Admittedly, it's sort of standardized these days, but there have absolutely been militaries that use the same ranks for both land and sea.
