Red's 'Foot in your ass' origin

I was just wondering if the producers/writers or Kurtwood Smith himself ever explained where the whole 'Foot in your ass' running gag came from and all the creativity behind it. I have heard people saying they will kick someone's ass but I have actually never heard anyone saying they will stick their foot in someone's ass let alone say it in so many creative ways ('My foot is about to drill a hole in your ass', 'How about I drive my foot into this thing called your ass?' etc.)
