Acne problems

I've been on TRT for almost a year & have been getting blood work done on a regular basis. My test is above average & my estrogen is on the lower end. However, my back & shoulders are pretty bad with acne & acne scaring. Any tips on what to do. I already use salicylic acid & have already used African soap... But no luck. TIA
