Need help interpreting results : low range total T and SHBG and normal free T.

38 years old guy here. 6ft tall and 210 pounds around 16% bf. I’m in a decent shape and I stay active. I go to the gym 3 to 4 times a week and I ski or mountain bike about 4 to 8 hours a week depending on the season.

Have a lot of low T symptoms… depression, no motivation, low energy levels, brain fog, no libido, no morning boners 75% of the time, hard time gaining mass but easily gaining fat… imsonia… I do have a cpap and I’ve been using it for over 6 years every night…I’m dialed in for my sleep apnea and I’m followed by a doctor for that and everything is good..but still, I wake up many times per night and I wake up tired and foggy every morning…I’ve been on seroquel and zoplicone for a long time to help me fall asleep but I stopped sleeping pills few years ago and I now take a few hit of my weed vape an hour or so before going to bed…I sleep better and I’m less foggy the next morning with weed vs with sleeping pills.

FYI, had a major depression in 2018… I went on SSRI (tried many) for about 2 years then stopped the SSRI…. Gained a lot Of weight during that period so I did the keto diet in 2021 (for a year) but the rapid weight lost due to a high calories deficit fucked my already fucked up hormones from SSRI big time…. I do have the feeling that my hormones never came back from these tough years of depression and restrictive diet… for the last 2 years I’ve been on a low carb, moderate fat and high protein diet and I eat at maintenance or slighlty above… my weight is stable but I’m still not feeling well for all the reasons above.

Here are the results from my bloodtest. The test has been done at 8h45 am in a fasted state.

Total T : 12.9 nmol/L (range 6.9 - 23.4)

Free T : 328 pmol/L (range 70 - 428 )

Bio available T: 8,2 nmol/L (range 2.3 - 14 )

SHBG : 19.1 nmol/L (range 13 - 89.5)

Albumine : 46.1 g/L (range 35 - 57)

Estradriol : no precise results… displayed result is < 70 pmol/L (range 0 - 121)

Prolactine : 6,9 ug/L (range 2.6 - 13.1)

FSH : 5.9 U/L (range 1.3 - 19.3)

LH : 2.9 U/L (range 1.2 - 8.6)

TSH : 1.8 mU/L (range 0.38 - 5.33)

The TRT clinic want me to go on a 100mg/week of T cyp splitted in 2 doses… a part of me wanna try it because I’m miserable and I feel like I’ve tried every lifestyle changes to get better…the other part of me hesitates because my results are low but still within range… any advice or opinion will be much appreciated! ✌️
