Currently In the ER after doing my TRT shot & wanted to save time and see if me doing it wrong was the cause.

For context my brother usually does my shots for me, he is on vacation currently and I did it myself for the first time, the first 2 weeks of shots went fine when he did it for me but when I did it today my heart rate immediately spiked down to 40, I got ringing in my ears, my body got extremely hot & felt like I could barley stand and was about to pass out, could this be because I injected too fast? Or at the wrong angle? I’m currently in the ER awaiting results, but am feeling somewhat better. (Note this couldn’t be anxiety due to the downward heart rate & that I am on anxiety medication) Thank you to all for any advice.
