Who do I give my doctors note to?

So I've been working flow for 6 weeks now and woke up today feeling horrible so I called in and said I was sick and wouldn't be able to come in.Since it is within my 90 days I got a doctors note when I went today.So I'm wondering who do I give my note to? Does having a note help at all?
