Anyone have experience with this club?

Wife an I are planning a trip to Italy and wanted to check out the local flavor. I'm having difficulty figuring out what they are asking for in these fields for registration, particularly the "residences" and "road". I don't see a place to add my email for contact and I wondered if one of these was meant for that. Or if they are asking country for the residences and address for the road?
Also, we aren't totally set on going to that club if anyone has a better suggestion. We have a free night or two in Napoli and Roma while we are there, so either of those locations would work.
