How to handle a broken agreement between two couples?

Scenario is, Anne and Bill are a couple, Claire and Dan are a couple. All four get along great and have played in various configurations. Everyone does not have to be present, twosomes and threesomes are okay (everyone communicates ahead of time, of course). Only rule is condoms-- for penetration between swapped couples and on toys.
A while back Bill and Claire got together for an evening. No problem...until some time later Bill lets slip to Anne that he and Claire didn't use a condom.
I'm Anne in this scenario. Im working through things with my own partner, but not sure what to do about sorting this out with Claire and Dan.
