I Know Basics of Speech Therapy but There’s an interesting problem

So I had a bad stutter back when I was elementary but I went to a speech therapist and it went away for the most part. So I know the basics of breathing in, pausing, and the words coming out of my mouth as the exhale. My problem now isn’t the typical stutter (e.g. where I an repeating the sound starting letter of a starting letter of a word). My problem now is an issue with the words coming out after I inhale and pause. When someone speaks to me/asks a question I can speak fine and it comes out properly. But other times (eg when I am having dinner with my parents and it’s quiet and I want to break the silence) the words won’t come out/it’s hard for me to get past the inhale and pause portion. So I don’t think it’s transcortical aphasia or any type of brain damage because I have had my brain scanned before and there is no damage. Plus it doesn’t happen all the time (maybe 40% of the time when it is silent). So does anybody have any advice or any videos to resolve this issue?
