My sleep clinic isn't very good. I'm seeing an ENT tomorrow to see if there are any issues, any advice on what to ask for?

My sleep clinic is bad already, but they've been going through turnover. I no longer have a doctor, and a nurse practitioner is filling in to handle the basics. Because of this it's going to be awhile until they are helpful again. I've been on my CPAP two years and it hasn't helped, without the CPAP I have very loud snoring, labored breathing. I also have been getting throat infections a few times a year. I'm hoping to get an ENT to check me out, but I'm not sure what to ask for, can anyone advise?
