[Routine Help] My skin (37F) is freaking out! (red/flaky/stinging/breakouts)

I had a routine that wasn't great, but was good enough. But then late last year two things happened: I got endo surgery (successful!) which seemed to change my skin (hormones/stress on body/who knows) my ride or die cleanser formula got changed last year and I ran out two months ago. I can’t find a new cleanser that doesn’t feel stripping. The past two months I've been trying different things for my uber sensitive skin and I'm using all CeraVe right now and it's a freaking nightmare - my skin is red, breaking out, rough patches, and stinging. I'm hoping the wonderful folks in this sub can help - I've been reading here and one other skincare subs for ideas, but alas no winners so far. I'm a bit desperate for additional suggestions.

About my skin:

  • Mid/late thirties so need hydration to keep it supple and very delicate skin barrier. 
  • Combo skin, but mostly dry/flaky (because I can't maintain the skin barrier).
  • Redness from old acne scarring (but it gets really bad when I can't maintain the skin barrier).
  • I only break out when either (1) I screw up the skin barrier, (2) too much niacinamide, or (3) I don't use any cleanser
  • I don’t currently use any acid or retinoids or toner or serum - my skin is too unhappy to even try, but I’ve used them in the past. 
  • I eat a low histamine diet and pretty healthy because of health issues - so I don’t think it’s a diet issue.

Current routine which is causing my skin to freak the heck out rn and is horrible:

  1. Garnier Micellar Water fragrance free (used this for a while, no reaction issues)
  2. Cerave hydrating facial cleanser (new to routine, I think this is wrecking my barrier)
  3. Cerave moisturizing cream (new to routine, I don’t think this is helping anything)
  4. In AM, I use Cerave AM Facial Moisturizing Lotion (I’ve used this on and off - its the only US sunscreen I don’t react to)
  5. In AM, I use Olehenrikson Banana Bright Eye Cream (used this for a while, no reaction issues)

Product sensitivities (this really limits my options, unfortunately but because of histamine/mast cell issues they are just non-starters):

  • No fragrance. At all. Not even any kinda of essential oil or botanicals. No masking fragrances either (a lot of Japanese products I've tried have these, and I haven't found a website yet that distinguishes between 'unscented' and 'fragrance-free')
  • No soy or wheat.
  • No avocado oil.
  • Low niacinamide in the overall routine (I keep overdoing it between multiple products)
  • My skin barrier seems to be uber needy
  • No on most US sunscreens - they are stingy and/or I break out, yes even physical ones and even the EltaMD ones

Products I've tried prior to Cerave - cleansers:

  • Osea Ocean Milk Cleanser - worked best of anything for a cleanser until they changed the formula last year
  • La Roche Posay Toleriane Hydrating Gentle facial Cleanser - loved this! Until using this plus Cicaplast Hand Cream (Mains) was Niacinamide overload
  • La Roche Posay Toleraine milky cleanser - felt greasy

Products I’ve tried prior to Cerave - moisturizers:

  • La Roche Posay - gave up on the line because of Niacinamide
    • Cicaplast B5 - love this when I can tolerate it, but still not ‘enough’ to heal my face when I get dry spots
    • Cicaplast Hand Cream (Mains) - this was amazing on my face and my face actually felt smooth and healthy and supple until the niacinamide overload
    • Lipikar Eczema cream - meh, goopy but didn’t do much for my face
    • Lipikar - multiple options - all meh similar to Vanicream and Cerave creams/lotions
    • Toleraine Dermallergo Moisturizing Cream - made my face sting, didn’t see benenfit
    • Tolerainen Double Repair Face Moisturizer (w/ and w/out SPF) - same, made face sting and didn’t see benefit 
    • Dermalogica - Has essential oils/botanicals
  • First Aid Beauty Ultra Repair Intense Hydration - has fragrance/botanicals
  • Vanicream Moisturizing Lotion - similar to Cerave, it’s like… whatever - heavy but I still get flaky patches
  • Bioderma Atoderm Intensive Balm - it’s fine, maybe better than Cerave and Vanicream? Maybe about the same.
  • Neutrogena Hydroboost (I use the fragrance free body lotion) - it’s fine, it’s a go-to, but my skin isn’t the happiest with it.
  • Vaseline - I use this on my lips and on my face sometimes over another lotion (most often the hydroboost) - it helps, but then it doesn’t feel like the rough patches actually heal, they just get moisture sealed in on top
  • The Ordinary Natural Moisturizing Factors HA - it’s fine, but too lightweight / doesn’t help heal rough patches
  • Rosehip oil - Both Pai and The Ordinary. I think it clogged pores more than helped. Didn’t feel like the moisture penetrated the skin.
  • Summer Fridays Rich Cushion Cream Ultra Plumping Moisturizer - stingy, didn't help rough patches
  • Clinique - the yellow one. Fine for summer when I don't have rough patches.
  • Clinique Moisture Surge 100h - seems to work for a short period of time, but if I put it on at night, my skin doesn't seem moisturized in the AM - and those rough patches don't seem to benefit much
  • Dermalogica - Has essential oils/botanicals
