How do I get over this fear and anxiety?

New to this subreddit but hopefully I can find some help. So I’ve been a sissy for about a year now, I wanna do the next step but don’t know what that is I like dressing up and wearing things under my guy clothes but have never come out to anyone, I’ve used chastity and anal devices like plugs and dildos but have always struggled with trying to stay clean when using them. Does anyone have any ideas what I should do next? How do I take the next step??? Thank you for any replies!
