I lost my child

I saw this last night before I went to bed so I wasn't able to check it thoroughly but I want to know your opinions and experiences if there's anything similar.
It's been a long while since I last played Sims 4, so when I opened up one of my households, I was shaken to see that my child is gone from the household and it was just me and my husband.
Could it be an error? I don't think that a child could be taken away by social services since he can basically feed himself if ever, the only thing I know that he could be taken away is because of being an orphan.
