How can I improve my gameplay beyond plat 3?

Hello, this and last season I have hit plat but always seem to constantly dip in and out of plat. Currently, my second account is 3137 elo and I cannot seem to progress more than 2 games into plat, clearly, my teammates aren't the problem as I play with a 5 stack and we scrim daily. If someone could please review my footage and give me some pointers on where I am going wrong to not progress above bottom plat 3.
Edit: the player bludog is no longer in our team.
