Need help with my scalp routine, still having flakes

I have been alternating between Selsun Blue and Ketoconazole every other day and conditioning the ends of my hair for about a month now. It has definitely made my scalp less itchy and greasy, but the problem is that I am still getting a decent amount of flakes and strands of hair with flakes on the end that fall out. Any tips?
