Colleagues! I need some outside POV. I’m a safety director at company where we use trailers. We have currently experience a couple of incidents where our trailers have had their tire pop out while our trucks are diving in the highway. The tires have popped out, jumped over guard rails, and crashed into incoming traffic.
I have looked at potential causes. We have a pre trip vehicle and trailer inspection lists which asks all drivers to report snowcat the wheel and axle, lug nuts, tire pressure and depth.
I’ve been wondering if perhaps it’s our lugs being tightened too much, perhaps they are loose, or we are hauling heavier stuff, causing our tires to bow out and eventually give out after repetitive use.
I’ve included some pictures from our last incident to see what you guys’s thoughts are. TIA
