SSA web site and others say to call SSA main # to request your CD (e.g. after denial as part of your appeal process).

I called today and they said to call my local office - that the procedure changed recently due to COVID, but the web site wasn't updated to reflect that.

Then I called the local office and THEY said the procedure is:

fill out and submit form SSA-3288 to the local office by fax, mail or in person:

https://www.ssa.gov/forms/ssa-3288.pdf

Hopefully I saved you the 90 minutes on hold that this cost me. ❤ Feel free to call your local office to check but don't waste time calling the main line
