Process for withdrawing from a course.

What's the process for withdrawing from a course? I have the handout about "valid reasons" (I keep accidentally injuring myself in this course which I took as a method to relieve my stress and anxiety but is doing the exact opposite. I can get medical documentation if need be and my instructor knows this.) However, I was under the impression that I had to approach them about withdrawing and them they further the process but I'm being told that's not the case although the instructor doesn't know what the process is. Does anyone know?
