Keyword Placement: Does a keyword NEED to be in the first few words of the article?

Hi! I run a comedy news site and am reteaching myself some SEO. My previous understanding is that an article about Dane Cook should have 'Dane Cook' as the first or first couple of words in the article (as well as title, subheds, meta description, images, etc.). Is this still the case? Never was?

I know you want keywords throughout the piece (without overdoing it), but how important is the keyword placement within the content and, if super important, where should they go?
