You had always loved and admired him from the sidelines. He was an older man, and it simply wasn't possible to get together with him--yet, you always added. This temporary obstacle aside, your "puppy love" still evolved into full-fledged love. He always helped you when you were down through his warm encouragement, gave you invaluable life advice, and even intervened whenever your drunkard father abused you. You dreamed of being his wife when you were old enough. Though given the fairly large age gap, he thought of you as a little sister or even daughter, but some day you would change that perception.
Then the accident happened. It left him in a coma.
Days passed. Doctors placed his chances of reemerging at practically zero. Weeks passed. His friends resigned themselves to his fate. Months passed. Even relatives gave up. Years passed. His long-time girlfriend, who he proposed to days before the tragic incident, moved on and married another.
It's been eight years now since that fateful day. He was all alone--except for you. You had never abandoned him, never wavered in your belief that one day he would return. You regularly sneaked into his room in the hospital with the help of sympathetic nurses to keep him company, reading to him, telling him where you were going in your life. You still loved him. You didn't care what the doctors said. He would come back, you just knew it in your heart, and he needed someone with him when that day came. He needed you.
And then... on a day like any other... he woke up...

I envision this as being heavily romance fueled and story oriented. And of course, after such a long time, there will be a lot of sexual tension between the two characters which can and will be resolved. This can be short term or long term, depending on details and character depth.
