Consultation next Wednesday

I have a consultation next Wednesday and I’m very excited. For the past two years my gynecologist has been saying I should get a reduction. And now I have the confidence to go to another surgeon after being denied when I was 18. I have UHC choice plus through my employer. I called them and they said it’s something that is covered, but it has to be pre-authorized. My Gynecologist has already faxed my referral letter. Is there anything else I should be prepared to bring?
