Just looking at the postcard, it looks like it's from before 1998 since it has a 5-digit space to write the postcode. For the new postcodes you're supposed to write the last two numbers underneath. Seems like the length has changed over the years! The Japanese Wikipedia article on postcodes in Japan has a lot of detail.
The postmark is from Shibuya Post Office and features the statue of Hachiko, Iris, the flower of Shibuya, Shibuya Post Office Building, and Mt. Fuji which you can see from the upper floors of the post office building.
