I emailed the woman who sends me the information and sent her a link to this thread. She doesn't have a Reddit account yet so here's what she told me to post for everyone wanting more information about this program.

This is who we are:

We are a group on Face book called Forget me not. We post addresses of any senior related care situations such as: nursing homes, meals on wheels programs, senior centers, Hospice, etc. After the address is posted in the group each Sunday you will then send as many cards or requested items as you are able to. Even if you can only send one card it will be a great help and very much appreciated. You will be sending directly to the care facility. All the addresses posted in our group come from places who have requested cards to be sent to their residents, participants, or patients so they will be expecting them. If you are not on Facebook and would just like to receive an email of the current address please feel free to message me for my email address and I will be happy to email you each week with the featured facility or care provider. We look forward to you joining our efforts to encourage a group of people who might other wise be forgotten.
