Clomipramine prescribed without general health check, is this a concern?

A psychiatrist prescribed clomipramine to a young patient. I noticed the following concerns:
- Patient did not have heart monitored prior to treatment (what if there was a serious heart issue that went unnoticed?);
- Patient did not have blood pressure measured (what if BP was dangerously low to begin with, considering clomipramine can lower BP?);
- Patient did not have depression scored (how do you know if there are improvements, semi-quantitatively speaking?)
