Dept. Administrators - Improvement Plans for Faculty?

Does anyone have an example of a template or a self-created form they've used for faculty improvement plans? My chair has asked that I put an instructor on one (I have not problem with this), but my college doesn't seem to have any formal system of documenting TIPs. I've found a lot of samples online but, predictably, they're all directed at HS teachers and they are complicated.
