survey creation help

Hi - My organization has a large number of BI users and I want to run a survey of their current usage, what they like, dislike, and what they would like to see if future iteration. My objective is to assess if there is an immediate gap that can be filled with existing tools or should we look for another advanced tools for BI (reporting) needs.
Any anyone run a survey like this and can share some insights? Thanks!
