In the once United States of America, especially in the region the Kingdom of Kiryas Satmar inhabits, "the Mob", or the La Cosa Nostra had significant control. Now, in a post flood America, the La Cosa Nostra seem to have taken a hold of the underground once again. The new La Cosa Nostra emerged from impoverished Italian immigrant neighbourhoods and quickly took power of the drug trade, weapons trafficking, extortion, and money laundering. They had already grown stronger than the small Mexican Cartels and the Jewish Mob (The Kosher Nostra), a seemingly untouchable criminal syndicate. Already major crime families have been established.

The Moretti: The Moretti are a traditional Mafia family, run by the Don Vincent Moretti. His Consigliere is Jerry Favaloro. They have over 90 Made Men and nearly 1000 associates.

The Del Golfo: The Del Golfo are a diarchic Mafia family, run by two bosses. They are the brothers Joey and John Del Golfo. They have 120 Made Men and over 1000 associates.

The 'Ndrangheta: The 'Ndranghetta are a union of small Mafia families all united under a Mafioso Council known as the 'Ndranghetta'. They have 100 Made Men and over 1200 associates.

The Vitozzi: The Vitozzi hare a 'Camorra' type family, with a more horizontal structure than the traditional Mafia style. They are led loosely by the Vitozzi family, but they hold over 20 Camorra families in their group. They have 70 Made Men and 700 associates.

The Sopranos: The Sopranos are the most prestigious family, having a long line all the way back before the flood. They are led by the Godfather Frank Soprano. His Consigliere is Silvio Dante. They have 200 Made Men and 800 associates.

All together they are known as the Five Families, the most powerful Mafia families in the entirety of Kiryas Satmar. They hold an absolute monopoly on organised crime within the Kingdom, being almost untouchable by various crime groups. If any rising families try and fight to become the famous "Sixth Family" they are quickly dismantled or absorbed. While the Five Families are technically all in competition with each other, they have an honour system established that will prevent each family from leaving the status quo.

1. Omertà: The code of silence.
2. Ethnicity: Only men of Italian descent through their fathers are allowed to become 'made men'. Associates have no ethnic limits.
3. Family Secrets: Members are not allowed to talk about their family business to non-members.
4. Blood for Blood: If a family member is killed by another member, revenge cannot occur unless the Boss gives permission.
5. No Fighting Among Brothers: From fist fights to knife fights.
6. Tribute: Every month members must pay the Boss his cut.
7. Adultery: It is the greatest insult to commit adultery with another family member's wife.
8. No Facial Hair: A made man is a Clean man.

For now the Five Families are in a 'cold war' type scenario. Neither family is taking the chance to try and depose the Sopranos from the title of Capo Dei Capi (The Godfather), not yet at least. However, there have been rumours of an alliance of the Moretti and the 'Ndrangheta brewing in the estates of the Mafia, but only time will tell...
