Weird penis change after masturbation/ejaculation.

I haven’t jerked off much within the past couple years because of some weird issues going on (maybe hard flaccid or peyronies). But anyway, every-time i masturbate now, my penis stays super tense at the base and doesn’t relax for a while. It looks super shrunk up and feels firm and tight. Do you know what this could be? I get hourglassing sometimes too, no pain though.
