Cycle Day 22, non stop spotting and this came out,should I go to er or not worth it

Iām having an irregular cycle with possibility of fibroids, went to the er CD9 because I was passing big clots and they gave me something to stop the bleeding and said to come back if it gets heavy again. The bleeding didnt stop but became like a spotting starting CD 12-13. This morning I found this when I went to pee. Iām getting frustrated and I dont know if I should go back or wait it out. I have a gyno appointment only on the 5th of may
