Recon question....

I have reconstituted many times, however, twice I have had vials/powder that was like clumpy rather than powdery and the issue was when I added the water, some clumps remained. Like those few pieces refused to dissolve and would get stuck on the sides of the vial. I'm wondering if they had a bit of moisture in them? Is there a way to make them dissolve?? Any help is appreciated.
