Physical therapy for PN yea or nay?

Anecdotally I have heard it both ways - physical therapy is helpful for PN (pudendal neuralgia), or the lions share of healing comes from pudendal ganglion or caudal block type interventional stuff. I don’t know if there is any strong literature on what in PT works and what doesn’t.

The suggestions given to me have been things like using the pelvic wand and also sitting on a tennis ball or using massage on the inner check perineal area (which I don’t think is correct or productive for everyone with the condition, in my case there is too much irritation). I’ve tried it but it just results in horrible, horrible flare ups and I was validated to hear from a doctor that isn’t necessary to do this.

Within the wand stuff, the advice has been to use the thin side to hit 4-8 o clock, using the clock metaphor, and also to work up to using the curved side to reach 2 o clock (the OI) but I can’t do it yet. I don’t know if that’s normal, but I don’t have too much internal pain or pain with intercourse or anything, and want to keep it that way, so I have no desire to rush it and get a backlash like I have with the ball.

just wanted to demystify what the PT for this is and check in on others experiences.
