Simple PF2e rules question

AC. When you add your proficiency bonus, does that mean someone wearing an armour that gives a 5 bonus and is trained in heavy armour that they would have an AC of 18 (with 10 dex)? Wouldn't that give increadibly high ACs even in only the mid game or a little before?
Also what does trained in unarmored defense mean I.E. Wizards.
