Currently I am running a Extinction Curse game and in the first session the team of 4 were fighting a rather strong enemy. The fighter and Barb are fighting the enemy and the fighter drops down hard to dying 2. The team manage to kill the enemy but can't help him cause no one bought a medicine pack. The fighter fails his roll and dies. The fighter is a friend of mine and got me into Pathfinder 1e and he says "maybe a doctor is brought in because of the commotion". And I listen to him. But part of me feels like I shouldn't have done it and let his character die. I want to be more true to the rules but I feel he's a bit overbearing because he would try to keep all his players alive.
What could've been the best course of action?
( This was our first game with PF2 but they knew it's a ruthless system )
