If my last passport expired in 2016 do I apply for a renewal or do I apply as if it’s a brand new application?

Title says it all. My passport expired in 2016 and it also had the wrong last name on it. How do I go about getting a new passport? I have to travel out of the country mid/late November I just found out and am at a loss for how to go about it. Thanks in advance for any help/guidance!
