Need help with Exercise/Diet Induced PVC’s and Palpitations

Hi all,

Hoping I can find some with similar experiences here. Been suffering from a sudden worsening of heart palpitations and PVC’s over the last few month as as expected going to the cardiologist was not very helpful in finding the cause.

6’0 190lb M26 here. Been losing quite a bit of weight over the last 1.5 years down from a high of 220 to around 183 back up to 190ish after a break in my diet. I’ve noticed recently that if I workout (weights 20min cardio 5-6 days per week) and combine with a modest calorie deficit at around 2500 calories per day I slowly start to get more and more symptoms that I haven’t had in the past when dieting. PVC’s will about double in frequency vs when I am not dieting and I can feel my heart pounding(forceful not fast) all day. The forceful heartbeats will sometimes cause a “whooshing” or “swishing” kind of a raspy feeling heartbeat. It’s hard to describe but it is extremely uncomfortable

I had been dieting from September to end of December 2022 losing about 20 pounds during that time. I was getting some increased PVC’s towards the end but nothing to unbearable. Took around a 3 week break on my diet and started up again at the end of January. Within about 2-3 weeks of starting my diet again the palpitations became unbearable. No clue why it’s different this time around but I feel like the change is too sudden not to be caused by some external factor.

Tried to push through it at first but it got so unbearable I had to mostly stop working out or dieting for the last 6 weeks. Started to feel normal again with rest and an increase in sodium and magnesium in my diet so I decided to start up again. After about a week back on my weight loss regimen now I can feel the symptoms beginning to come back so I am at a loss of what to do.

The only things I can think of that were different this time around were that I got sick with what I presume was a cold (no signs of pericarditis in any of my tests done so far though) or that I may have cut my sodium too low. I went down to around 1400mg per day. In my most recent blood tests my sodium level was at the very bottom of the reference range for normal (136). Potassium was right in the middle of the range. So wondering if I could just be very sensitive to slight electrolyte Imbalances?

EKG and Holter came back normal with holter only showing about 0.38% pvc burden. Blood tests all came back normal.

Getting an echo and stress test done in about 10 days but Im doubtful they will show anything helpful.

Has anyone else had the same thing and found an answer other than just taking a beta blocker to calm the heart down a bit? If so what did you do?(already taking 400mg magnesium and coq10)I realize it’s probably not anything that’s going to kill me but it is so annoying and keeping me from losing the last 20lbs or so I would like to.

EDIT: Symptoms are worse for rest of day after working out. During workout I can kind of feel the pounding but it’s not enough to stop me from being able to complete the workouts. PVC’s are almost exclusively at night while laying flat
