What causes shots to entirely miss?

Post Body

I was using a mini with an 8x on a guys head at 200m. The player was not moving, I was not moving. I was centered on the head.

The shot missed still. I could not have changed the outcome.

What causes this?

Edit: I don't know how to or feel like learning how to upload the reply that may or may not accurately portray what took place. Just take my word for it that I was doing everything I could have to hit that shot and it simply didn't happen. Some sort of server issue or randomness is the best explanation I've received. Thank you.
