Why cant I spectate friends in QP or Comp?

What does the spectate option actually do?
I can never seem to get it working if a friend is in a match I right click on them and "Spectate" is nearly always greyed out.... is this feature only for custom 5v5 or something? I tried google but answers are from 12 months ago that it was removed for some reason
