Has anyone else had their nade just vanish as Ana?

It's happened to me twice now - first time I thought it was a glitch, but the second time I went back through my replay and confirmed it just never appeared at all.
My character model does the motion of throwing, and my cool down timer resets so it's definitely 'happening', it's just not working right.
Have you experienced this too?
