What are some red flags when signing on with a contracting company in a SNF setting?

Hello fellow OT crew! I'm in a bit of a pickle and would appreciate any and all input. I recently received an offer letter from a known therapy contracting company. However, after reading the offer letter it states that my hourly wage may change at their discretion & that I'm considered an ON - CALL COTA/L (which I didn't agree to). It further states that the facility I chose (which is close to my home) is also subject to change at their discretion. None of this information was mentioned during my 2 video interviews. 🙃
