The survey received nearly 180 responses with just blows my mind.
There were two instances of responses that appeared to be duplicates of another. I highlighted these two with red in the excel sheet. I did not remove/delete any responses.
I downloaded the spreadsheet of responses and removed all of the usernames:
Unfortunately there wasn't an easy way to add everyone to the Google Form itself to see the responses, because in order to see the results on the Google Form you have to be an "editor" which allows you to delete the responses.
So instead I was forced to create an imgur album of screenshots of the Google Form. The limitation of is that you can't easily see what people typed in for the "Other" sections. The spreadsheet is more detailed.

Some interesting observations:
77% of respondents said that Phenylpiracetam improved their energy.
75.3% of respondents said that Phenylpiracetam improved their focus.
Just over 1/3 of respondents said that Phenylpiracetam could lead to short-term insomnia. I could see how that could happen if you take it later in afternoon/evening.
89.3% of people responses "No" to the question "Do you feel that phenylpiracetam usage in the past has left you with any permanent side effects?" almost everyone else selected "Maybe". But, out of that group, some of them selected "Yes" to the question asking if they would take phenylpiracetam again. And, some others who selected "Maybe" also did not report/select any long term side effects. That makes me wonder if a portion who selected "maybe" did not fully understand the question. Ultimately, I didn't delete any of the responses, so you can draw your own conclusions on this part via the spreadsheet.
Please remember that the spreadsheet results are not medical advice. The results are a collection of anecdotes. This is not a proper study, just a community survey/poll. I am not a doctor, and I am also not a statistician. I can't personally say how many, if any, of these may be bots, or such.
