If you have a restraining order on someone, what happens when you choose to enter that distance, knowingly or unknowingly?

Purely a hypothetical, but let's say Person A gets a restraining order of 500 yards to Person B. Person B is at a mcdonalds, Person A later sees Person B at mcdonalds. What happens? Can punishment still be dealt if Person B didn't know? If Person A entered second? What if person A knowingly closes distance?
