When asking for a raise at work, how do you determine your new worth?

I know there's many factors that will play into this answer. However, I've always worked for companies that evaluate you yearly and based off of a performance review gives you a raise of a percentage of your salary/hourly wage. Well since taking a new job (it's salary), I've put in ample amounts of hard work and I'm 100% certain my boss recognizes it. In fact, he constantly tells me that he's preparing me to apply for the next position in the field (his position, different district) once an opportunity presents itself. But that opportunity can come in 2 weeks or 2 years and in the meantime I would still like to be able to discuss my self-worth and don't know where to begin or how to initiate the conversation once I'm ready to do so.
