is there any real example to an emergency syringe jab? or is it all video game fictional mumbo jumbo

For example, would someone with severe allergies going into shock haphazardly jab an epi-pen into their thigh? Or are any and all effective administrations of medicine by needle/syringe always done carefully into the bloodstream and not haphazardly?
